This hybrid role combines time in the office with hands-on field oversight, offering the perfect balance of leadership and clinical engagement.

What You’ll Do

• Lead, mentor, and support home health clinicians in the field

• Provide clinical oversight and supervision to ensure high-quality, compliant patient care

• Review and support OASIS documentation for accuracy, timeliness, and regulatory compliance

• Serve as a clinical resource for staff questions, case reviews, and problem-solving

• Collaborate with leadership to improve workflows, quality outcomes, and clinical processes

• Promote a culture of accountability, collaboration, and professional growth

What We’re Looking For

• Strong OASIS experience required

• Home health experience required

• Solid clinical background with demonstrated leadership or supervisory experience

• Excellent communication, organization, and coaching skills

• A proactive, solutions-oriented mindset

• Passion for developing clinicians and improving patient care
