About the Role

We are seeking a compassionate and motivated PRN Medical Social Worker to join our home health team in the North Las Vegas area. In this role, you will support patients and families in their homes by providing assessments, care coordination, and ongoing support as part of an interdisciplinary healthcare team.

What You’ll Do

• Conduct psychosocial assessments and implement individualized care plans

• Provide ongoing support, education, and counseling to patients and families

• Assist patients and caregivers with self-care strategies and community resources

• Coordinate services with physicians, nurses, and other care team members

• Document patient progress and provide timely reports

• Advocate for patients to ensure continuity and quality of care

Qualifications

• Active Medical Social Worker license in the state of Nevada

• Master’s degree in Social Work (MSW)

• Minimum of one (1) year of supervised post-licensure experience in a healthcare setting

• Home health experience preferred

• Ability to pass a background check
