• Perform in-home physical therapy evaluations

• Design and implement individualized treatment plans

• Help patients achieve mobility, safety, and functional goals

• Coordinate with interdisciplinary teams for holistic care

• Document accurately with the help of AI-enhanced tools

What You Bring

• Active Nevada PT license

• Home health experience (required)

• CPR certification

• Strong clinical judgment and communication skills

• A dependable vehicle, valid NV driver’s license, and a passion for patient-centered care
