What You’ll Be Doing:
Lead Patient Intakes: Collect and organize referral and medical documentation with precision.
Coordinate With Providers: Collaborate with doctors, nurses, and care teams to ensure continuity.
Handle Insurance & Authorizations: Verify coverage and secure timely approvals to avoid delays.
Maintain Accurate Records: Enter and manage patient information in EMRs and digital systems.
Stay Compliant: Uphold HIPAA and privacy standards with every piece of information.
Support Patients & Families: Communicate clearly, professionally, and with compassion.
What You Bring to the Role:
Education: High school diploma required; associate’s or bachelor’s degree preferred.
Experience: Minimum of 2 years in a healthcare environment — Home Health experience strongly preferred.
Skills & Knowledge: Proficient in EMR systems, medical terminology, and insurance procedures.
Detail-Oriented: You catch what others miss and keep processes running smoothly.
Strong Communicator: Confident and compassionate with patients, families, and providers.
Adaptable: Comfortable with shifting priorities and occasional evening/weekend needs.
